Are you a new client, or would you like to be one? After scheduling your first appointment at Haven Animal Hospital, please fill out both the Welcome and Vaccination History forms and email them to [email protected] PRIOR TO your first visit. This lets us record our new pets and owners into our management software before you even walk in the door to ensure efficiency and effectiveness.
